# Entering dechallenge and rechallenge information ### Enter dechallenge information Dechallenge information is entered as the combination of *Drug – Action taken* (Drug withdrawn) and Reaction - *Outcome*. ### Enter rechallenge information Rechallenge information is recorded for each drug.
