# Entering dechallenge and rechallenge information

### Enter dechallenge information

Dechallenge information is entered as the combination of *Drug – Action taken* (Drug withdrawn) and Reaction - *Outcome*.

### Enter rechallenge information

Rechallenge information is recorded for each drug. If the field *Was rechallenge performed?* is marked “Yes”, a new field called *Rechallenge information* appears, where you can enter the results of each rechallenge.
